CareHarmony is looking for a Remote Licensed Practical Nurse (LPN/LVN) to join our team as an Intake Coordinator for the Day Shift. In this role, you will work directly with providers to implement value-based care management by conducting comprehensive patient outreach via phone.
What You'll Do
- Accept patient transfers from the Enrollment team to conduct preliminary health assessments.
- Resolve patient questions and create an open dialogue to understand their care needs.
- Identify and coordinate community resources to benefit patient care plans.
- Provide patient education and improve health literacy regarding chronic condition management.
- Assist with medication management, including identifying adherence concerns and coordinating refills.
- Ensure timely delivery of services such as Home Health, DME, and Home Infusion.
- Adapt to a fast-paced environment and spend most of your shift engaged in phone communication.
What We're Looking For
- Active Compact or Multi-State LPN/LVN license.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.
- Ability to speak clearly and listen attentively by phone for extended periods.
- Ability to understand, process, and take thorough real-time notes on telephone conversations.
Nice to Have
- Experience with Epic EHR systems.
- Bilingual communication skills.
- Additional Single State licensures.
