Responsibilities
- Receive patient referrals from enrollment staff to perform initial health evaluations for new network participants.
- Address patient inquiries and foster clear communication to assess individual care needs.
- Locate and arrange access to community services that support patient well-being.
- Educate patients on managing chronic health conditions and improving health understanding.
- Support medication oversight by identifying potential issues, promoting adherence, and managing refill coordination.
- Ensure prompt arrangement of essential care services such as home health, durable medical equipment, and home infusion.
- Adapt efficiently to a dynamic, high-volume work setting with a primary focus on telephone-based patient interaction.
