As a Registered Nurse joining our team, you'll have the opportunity to support respiratory patient needs remotely while collaborating with respiratory healthcare professionals to transform specialty medicine. Responsibilities: - Introduce patients to continuous care programs and explain comprehensive service benefits - Collaborate with Respiratory Therapists and pulmonary providers for integrated care approaches - Instruct patients on in-home monitoring device usage and frequency - Develop personalized care plans through telecommunication consultations - Recognize and mitigate potential patient success obstacles - Deliver targeted education regarding chronic pulmonary conditions - Conduct regular patient interactions to review readings and monthly care plan updates - Serve as primary contact for non-urgent patient requirements - Assist patients in healthcare navigation, including appointment scheduling and specialist coordination - Monitor and evaluate real-time home device readings during business hours - Adhere to established protocols for documenting device trends and associated symptoms - Escalate concerning readings/symptoms per provider communication guidelines - Cultivate meaningful patient relationships that foster trust and motivational health improvements Requirements: - Active, Unrestricted RN License in Georgia, Florida, Texas, or SC (Compact License preferred) - Minimum three years adult health experience - Background in adult chronic health conditions preferred - Diverse Electronic Medical Record system experience - Strong computer skill proficiency - Advanced problem-solving and multitasking capabilities - Superior oral and written communication skills - Strong collaborative teamwork abilities - Successfully pass background check and drug screening - Comfortable with remote yet collaborative work environment Benefits: - Comprehensive Healthcare Coverage (Medical, Dental, Vision) - Life Insurance options - Unlimited Paid Time Off - Professional Training and Development opportunities - Full-Time position with salary range of 55-65K
