CVS Health is hiring a Registered Nurse Field Case Manager to join our team serving Wayne and Macomb Counties in Michigan. In this role, you will be responsible for assessing, planning, implementing, and coordinating case management activities for members eligible for both Medicare and Medicaid. What You'll Do
- Develop a proactive plan of care to enhance short and long-term outcomes and member wellness.
- Use clinical tools and data review to evaluate member needs and benefits.
- Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers.
- Conduct assessments considering information from various sources to address all conditions impacting functionality.
- Use a holistic approach to assess the need for referrals to clinical resources and interdisciplinary team members.
- Collaborate with your supervisor and key stakeholders to overcome barriers in meeting goals.
- Utilize case management processes in compliance with regulatory and company policies.
- Utilize motivational interviewing skills to ensure maximum member engagement.
What We're Looking For
- Minimum 3+ years clinical practical experience as a Registered Nurse (RN).
- Active and unrestricted Michigan RN licensure.
- Willing and able to travel up to 25%-50% of the time in Wayne and Macomb Counties.
- Reliable transportation required.
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint.
- Effective computer skills including navigating multiple systems and keyboarding.
- Confidence working at home as an independent thinker, using tools to collaborate and connect with teams virtually.
- Excellent analytical and problem-solving skills.
- Effective communications, organizational, and interpersonal skills.
- Ability to work independently.
- Associate's Degree required.
Nice to Have
- 2+ years of experience in case management, discharge planning and/or home health care coordination.
- Behavioral health experience.
- Bilingual in Spanish or Arabic.
- Certified Case Manager.
- Additional national professional certification (CRC, CDMS, CRRN, COHN).
- Bachelor's degree preferred.

Work Mode
This is a hybrid role. The position is remote but requires 25-50% travel within Wayne and Macomb Counties in Southeast Michigan.
