Ochsner Health is hiring a Home Infusion Remote Medication Access Specialist to support our providers and ensure patients can successfully access and adhere to their prescribed therapies. You will play a critical role in managing medication compliance, navigating financial support and authorization processes, and maintaining accurate medication histories within our electronic health record system.
What You'll Do
- Process patient on-boarding for home infusion services.
- Coordinate with providers and patients to assess eligibility for financial support programs.
- Navigate the complete medication authorization process.
- Obtain and clarify patients' home and chronic medication lists.
- Use the EHR to input and maintain accurate patient medication lists.
- Communicate discrepancies identified during medication history reviews with the pharmacist and clarify details with patients and prescribers.
- Collect patients’ prescription benefit information.
- Direct patient or family questions outside the medication history scope to the appropriate care team members.
- Provide high-quality customer service to patients, family members, and the care team.
- Document all findings comprehensively in the EHR for seamless provider collaboration.
- Adapt communication and approach to specific patient populations with respect for privacy and individual needs.
What We're Looking For
- High School Diploma or equivalent.
- 2 years of experience as a pharmacy technician, in medical intake, or as a healthcare employee working directly with medications.
- Medication Therapy Management Certification from the Pharmacy Technician Certification Board (PTCB) OR a Medication History Certificate from ASHP, which must be obtained within 6 months of hire.
Nice to Have
- Experience working with insurance companies to process medication authorizations.
- Experience working within outpatient clinics and processing medication refill requests.
- Direct experience taking patient medication histories.
