About the Role
This role is responsible for building trusted relationships with healthcare providers to enhance care delivery using data-driven insights and proactive support strategies.
Responsibilities
- Develop and maintain strong partnerships with healthcare providers
- Use data analytics to identify opportunities for improving patient care
- Collaborate with providers to implement care improvements
- Serve as a primary point of contact for provider inquiries
- Monitor provider performance metrics and trends
- Deliver actionable insights to support clinical and operational goals
- Coordinate with internal teams to resolve provider issues
- Facilitate onboarding for new providers
- Conduct regular check-ins to assess provider needs
- Support adoption of technology tools used in care management
- Track and report on provider engagement outcomes
- Identify barriers to care delivery and recommend solutions
- Promote best practices in patient engagement and care coordination
- Assist providers in interpreting performance data
- Ensure providers understand program requirements and resources
- Respond to provider feedback and escalate when necessary
- Maintain accurate documentation of provider interactions
- Participate in training and development programs
- Stay current on healthcare industry trends and regulations
- Contribute to process improvements within provider support workflows
Nice to Have
- Master’s degree in public health, business administration, or healthcare administration
- Experience in a startup or rapidly scaling organization
- Background in clinical care or direct patient interaction
- Certification in healthcare quality or patient safety
- Familiarity with Texas-specific healthcare markets
