Responsibilities
- Provide individualized nursing care in the patient's home setting
- Carry out medical care plans as directed by physicians and maintain accurate, compliant records in the designated system
- Deliver prescribed medications, treatments, and therapeutic interventions
- Conduct patient evaluations and coordinate ongoing care needs
- Educate patients and families on health management and provide necessary training
