IMPaCT Care is hiring a Physician Partner / Indiana

About the Role

IMPaCT Care is seeking board-certified physicians (MD/DOs) to join as Physician Partners. In this short-term, high-impact role, you will help identify health conditions and care gaps, stabilize medical concerns, and connect patients to longer-term primary care solutions.

What You'll Do

  • Dedicate 5–10 hours per week to real-time telehealth visits with patients referred by Community Health Workers (CHWs), including consults and prescribing medications when appropriate.
  • Collaborate with CHWs to evaluate patient needs and determine eligibility for Community Health Integration services.
  • Provide clinical consultation and oversight to CHWs as they support patients with social needs and healthcare navigation.
  • Promote preventive care and overall well-being through education and evidence-based strategies.
  • Maintain accurate patient records, including reviewing and affirming documentation created by the patient’s CHW.
  • Respond to patient questions, chart notes, and administrative messages as they transition to a primary care home.
  • Provide occasional follow-up visits to address urgent or time-sensitive medical needs.
  • Complete required training and adhere to company policies and best practices.

What We're Looking For

  • Active, unrestricted MD or DO license and current residence in the United States.
  • Minimum of 2 years of consecutive experience in family medicine, internal medicine, geriatrics, preventative medicine, or other primary care settings.
  • Passion for social medicine and addressing the social needs that impact health.
  • Ability to operate effectively as an independent provider with strong diagnostic and problem-solving abilities.
  • Experience providing clinical support and oversight to CHWs or other allied health professionals.
  • Skilled in navigating multiple computer screens and proficient with tools such as EMR, text expanders, Gmail, Google Calendar, Zoom, and scheduling platforms.
  • Location preference: We are actively seeking providers in IN, PA, MD, CA, NY, and NJ.
