Responsibilities
- Deliver specialized care in Physical Medicine and Rehabilitation as a consultant within rehabilitation settings
- Serve as a clinical leader and representative within the facility, promoting collaboration across care teams
- Partner with primary care provider groups to manage common conditions seen in skilled nursing facilities, including stroke, neurodegenerative diseases, and post-surgical orthopedic recovery
- Collaborate with physical therapy, occupational therapy, and speech-language pathology teams to support functional improvement and patient well-being
- Monitor therapy patients regularly to evaluate and document progress
- Evaluate and assist in managing medical issues that interfere with therapy, such as joint pain, nerve pain, muscle stiffness, and neurologic bowel or bladder dysfunction
- Participate in interdisciplinary team meetings to integrate medical and therapeutic care planning
- Contribute to team development through educational initiatives, including monthly learning sessions, virtual regional gatherings, and annual in-person events

Team
Interdisciplinary rehabilitation team in skilled nursing facilities
Responsibilities
- Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team
- Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside your PT, OT, and SLP teams to improve patient function and quality of life
- Follow therapy patients to track their progress in therapy
- Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!
- Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine
- Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues
