Responsibilities

• Evaluate patients and create individualized treatment plans based on assessment outcomes
• Analyze physician referrals and patient medical histories to determine appropriate therapy interventions
• Deliver hands-on care following established clinical protocols and care standards
• Record patient progress, clinical findings, and instructions for patients and caregivers
• Supervise and oversee care delivered by therapy assistants and rehabilitation technicians
• Develop detailed plans of care, including measurable goals and structured treatment programs
• Carry out prescribed therapy programs and administer necessary therapeutic treatments
• Maintain clear communication with patients, families, physicians, and insurance providers
• Ensure all documentation is completed promptly, including evaluations, progress notes, and incident reports
