P4 Physical Therapy is hiring a Physical Therapist

Responsibilities

  • Evaluate patients and create individualized treatment plans based on assessment outcomes
  • Analyze physician referrals and patient medical histories to determine appropriate therapy interventions
  • Deliver hands-on care following established clinical protocols and patient care standards
  • Record detailed notes on patient progress and provide clear instructions to patients and caregivers
  • Supervise and oversee treatment provided by therapy assistants and rehabilitation technicians
  • Develop written care plans with measurable goals and structured programs to achieve them
  • Carry out prescribed therapy programs and administer necessary therapeutic treatments
  • Maintain clear communication with patients, caregivers, physicians, and insurance providers
  • Ensure all documentation is completed promptly, including evaluations, progress reports, discharge summaries, and incident reports
