Sierra Home Health and Hospice is looking for a Physical Therapist to deliver patient-focused care in the homes of our community in Truth or Consequences, NM. You'll work independently with the support of a tight-knit clinical team, building meaningful relationships and providing personalized healthcare.
What You'll Do
- Deliver physician-ordered physical therapy services in patients' homes
- Evaluate patient function and create individualized care plans
- Conduct initial assessments, reassessments, and plan of care revisions every 30 days
- Supervise Physical Therapy Assistants and Home Health Aides as needed
- Educate patients and caregivers on exercises and home care routines
- Collaborate with the care team and physicians on treatment adjustments
- Prepare timely and accurate clinical documentation, including OASIS
- Contribute to quality initiatives, in-services, and patient care conferences
What We're Looking For
- Current licensure as a Physical Therapist in New Mexico
- Graduate of an accredited Physical Therapy program
- Strong assessment and care planning skills
- Ability to work independently and manage a home-based caseload
- Excellent communication with patients, families, and team members
- Valid driver’s license and reliable transportation
