Responsibilities
- Coordinate with doctors and healthcare team members to develop patient care plans
- Assess and prioritize incoming patient calls based on medical urgency
- Conduct in-person home visits to monitor and evaluate patient health status
- Deliver personalized emotional and physical support tailored to patient needs
- Partner with caregivers to ensure consistent, high-quality care delivery
- Support in the administration of prescribed medications
- Manage acute symptoms that arise during end-of-life stages
- Communicate critical changes in patient condition to physicians when required
- Guide patients and families through necessary documentation processes
- Keep thorough and up-to-date patient records
- Offer emotional support to family members preparing for loss and after death
