Included Health is looking for a Pediatric Behavioral Health Nurse Care Manager to join our Clinical Strategy and Services Clinical Team. This is a full-time remote position where you will manage care for pediatric patients with behavioral health needs.
What You'll Do
- Develop and implement personalized care plans for pediatric patients
- Coordinate behavioral health services and resources
- Monitor patient progress and adjust care strategies as needed
- Collaborate with healthcare providers and families to ensure integrated care
- Advocate for patients to optimize health outcomes
What We're Looking For
- Active RN license with pediatric behavioral health experience
- Proven experience in care management or case coordination
- Strong clinical assessment and patient education skills
- Ability to work independently in a remote setting
- Excellent communication and collaboration abilities
