Responsibilities
- Manage a panel of complex pediatric patients – Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
- Work in a multidisciplinary team – Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model.
- Empower families – Educate and support parents to navigate their child’s health journey with confidence, ensuring long term health success.
- Deliver high-quality virtual care – Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
- Ensure continuity of care – Step in when needed to bridge care gaps, coordinating prescriptions, and streamline referrals, to remove barriers to care.
Requirements
- Minimum 5 years’ experience with a pediatric population
- Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C)
- Hold an unrestricted NP or PA state license, including board certification
- Active NJ, NC, FL, or NY licensure is required
- Experience and comfort with children with complex medical needs
- Strong desire to improve the quality of life for pediatric patients and their caregivers
- Ability to lead and guide a clinical team
Nice to Have
- Community Based Health or Complex Care experience
- Experience with Medicaid and/or Community health
- Virtual care experience
- Bilingual Spanish
- Additional license in GA or DC

Compensation
The compensation for this role starts at $115,000 in addition to annual bonus and competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
