Lifecare Home Health Family is looking for a Patient Care Advocate to develop strategic business partnerships and drive referral growth for our hospice programs. In this role, you will serve as the primary agency liaison within an assigned territory, connecting with physicians, care facilities, and community organizations to promote timely, high-quality care for patients and caregivers.
What You'll Do
- Develop, implement, and evaluate quarterly and annual territory plans to achieve growth goals.
- Build and maintain business relationships by making effective sales calls and presentations to targeted referral sources, including physicians, facilities, discharge planners, and community agencies.
- Meet with patients and families to discuss home care services, address individual needs, and manage expectations.
- Update and maintain customer and competitor profiles, log daily call activity, and document follow-up plans.
- Participate in marketing efforts such as physician calls, lunch and learns, health fairs, and educational programs.
- Prepare monthly tracking reports on referral sources and keep the agency informed of key marketplace changes.
- Promote the hospice’s philosophy and administrative policies while providing counsel on community needs and program development.
- Communicate effectively with attending physicians, hospice physicians, patients, family members, and internal team members.
- Carry out other duties as assigned by leadership.
What We're Looking For
- A Bachelor’s degree in marketing or a related field, or equivalent professional experience.
- A minimum of two (2) years of experience in hospice sales or a related industry with a proven record of achieving incremental growth in direct sales.
- Experience delivering high-quality outcomes and growth.
- Skill in establishing and maintaining professional and effective relationships with internal and external teams.
- Proficiency in using Microsoft programs for communications, presentations, database tracking, reporting, and spreadsheets.
- Excellent oral communication skills.
- A self-directed individual who can work with little supervision.
- Reliable transportation and valid auto liability insurance.
