What You'll Do
Deliver skilled therapeutic care directly in patients' homes, focusing on improving speech, language, cognitive-communication, and swallowing abilities. Conduct thorough evaluations and design individualized treatment plans that respond to each patient’s unique needs. Act as both clinician and advocate, guiding patients and families through recovery with empathy and expertise.
Work closely with nurses, therapists, and case managers to coordinate care and ensure continuity. Use digital tools to maintain accurate, timely documentation and communicate effectively across the care team. Take ownership of your caseload with structured independence, supported by intuitive technology and responsive systems.
Requirements
- Hold an active, unrestricted California license as a Speech Therapist
- Maintain current BLS/CPR certification
- Possess a valid California driver’s license and reliable transportation
- Demonstrate strong critical thinking and organizational abilities
- Show consistent empathy, leadership, and interpersonal effectiveness
- Work independently with minimal supervision
Preferred Qualifications
- Familiarity with state and federal regulations governing home health services
- Background in home care or high-acuity patient management
- Proven experience collaborating within interdisciplinary teams
- Understanding of case management principles

Work Environment
This role operates entirely onsite, serving patients in Marin County, specifically San Rafael. While the position is part-time, it offers high flexibility in scheduling and strong operational support. You’ll use a web-based charting system and communication platform on company-provided devices, ensuring efficiency and compliance.
