Responsibilities
- Respond to requests for urgent medical, behavioral, or social support by traveling to patients' homes or locations within the community.
- Collaborate as part of a multidisciplinary care team to deliver skilled, compassionate care to pediatric patients and their families.
- Address urgent health needs in coordination with clinical team members, delivering therapeutic interventions under physician direction.
- Support the onboarding of new patients by assisting with clinical intake assessments and facilitating initial visits.
- Engage with patients following hospital discharge or emergency department visits to reduce readmission risks and address underlying causes.
