Responsibilities
- Deliver high-standard preventive, acute, and chronic medical care through a fully virtual platform
- Enhance the quality of remote patient interactions by offering empathetic and impactful consultations
- Serve as the primary care provider for an assigned group of patients
- Direct patients to specialist providers or emergency services when medically necessary
- Engage in population health efforts focused on at-risk patients, including annual wellness visits, closing care gaps, care transitions, and risk documentation
- Assist patients shortly after hospital discharge to reinforce treatment comprehension and lower readmission rates
- Conduct ongoing self-assessment to maintain compliance with clinical protocols and industry standards

- Must commit to 40 hours weekly, with 36 clinical hours scheduled between 9 PM and 7 AM Pacific Time
- Proficiency in Spanish is preferred
- Prior experience with EPIC electronic medical record system is required
- Must be currently enrolled or eligible for enrollment as a Medicare provider
- A clean background check and unblemished malpractice history are mandatory
- Work is conducted from home
