Responsibilities
- Own, track, and manage completion of clinical and operational tasks across all Pomona Valley clinics
- Establish timelines, accountability structures, and follow-up processes, and maintain tracking systems and reporting for task completion
- Coordinate with site leadership to ensure execution of priorities, and identify and close execution gaps
- Support development and implementation of standardized clinical workflows and process improvements
- Support HEDIS, STAR, and quality performance initiatives
- Assist with risk adjustment, documentation accuracy, audits, chart reviews, and compliance workflows
- Track performance trends and escalate issues to leadership
- Assist initiatives to improve patient retention, access, growth, and continuity of care
- Act as liaison between clinical teams and IT, supporting EMR and clinical system implementation, adoption, and troubleshooting
- Partner cross-functionally with clinical, operations, quality, and IT teams to align clinic sites and implement regional priorities
Requirements
- Active LVN license in California
- 5+ years of clinical operations or ambulatory experience
- Experience with quality programs such as HEDIS or risk adjustment
- Strong organizational and execution skills
- Proficiency with EMR systems and Microsoft Office
Nice to Have
- Experience in managed care, Medicare, or Medi-Cal environments
- Experience supporting multi-site operations
- Bilingual English/Spanish
