Practicebetter is looking for an Occupational Therapist – Integrated Reablement to support adult social care delivery. In this role, you will conduct timely initial assessments and deliver six-week tailored rehabilitation programmes, managing hospital discharges and community referrals to prevent, reduce, or delay long-term care needs.
What You'll Do
- Manage a varied caseload of hospital discharges and community referrals.
- Complete risk-assessed home visits and provide clinical oversight for reablement plans.
- Direct and support Reablement Officers delivering care programmes.
- Work collaboratively with a strong multidisciplinary team to achieve positive outcomes.
What We're Looking For
- HCPC registered Occupational Therapist.
- Minimum 2 years’ experience post-qualification (Band 6 level or equivalent).
- Proven experience in case work and physical rehabilitation in a busy environment.
- London-based for urgent case management.
- Ability to work flexibly: Monday–Friday, some remote work, and office-based two days/week.
