About the Role
The occupational therapist will evaluate patients in their homes, develop individualized care strategies, and support progress toward functional goals, focusing on safety, mobility, and self-care.
Responsibilities
- Conduct comprehensive assessments of patients' home environments and functional abilities
- Develop and implement customized treatment plans to enhance independence
- Train patients in adaptive techniques for activities of daily living
- Recommend modifications to home setups for improved safety and accessibility
- Collaborate with physicians and other healthcare providers on patient progress
- Document patient evaluations and treatment outcomes accurately
- Monitor patient improvement and adjust care plans as needed
- Educate families and caregivers on supportive strategies
- Ensure compliance with clinical and regulatory standards
- Manage patient caseload efficiently within scheduled visits
- Provide therapy services to individuals across various age groups
- Focus on improving motor, cognitive, and perceptual skills
- Assist with identifying and obtaining assistive devices
- Promote patient engagement and motivation throughout recovery
- Maintain professional licensure and continuing education
- Adhere to infection control and safety protocols during home visits
- Respond promptly to changes in patient condition
- Support transitional care from hospital to home
- Use clinical judgment to determine discharge readiness
- Uphold patient confidentiality and ethical standards
