Chime is looking for an Occupational Therapist to join its Adult Social Care team in Ceredigion, Wales. In this role, you will provide person-centred support within a multidisciplinary team, helping individuals live safely and independently in their own homes.
What You'll Do
- Complete Care Act-compliant functional assessments for adults with physical disabilities, long-term conditions, and complex needs.
- Develop and implement reablement plans to promote independence and reduce reliance on long-term care.
- Advise on major and minor adaptations, including Disabled Facilities Grants (DFG), and recommend assistive technology solutions.
- Undertake Moving & Handling risk assessments and provide training to carers and staff.
- Support hospital discharge pathways through timely assessments and coordination of community services.
- Work collaboratively with social workers, physiotherapists, housing teams, and other professionals to deliver holistic care.
- Maintain accurate case records and produce reports for panels and funding applications.
What We're Looking For
- HCPC registration as an Occupational Therapist.
- Previous experience in Adult Social Care within a local authority or NHS setting.
- Strong knowledge of the Care Act, safeguarding, and DFG processes.
- Ability to manage a varied caseload and work autonomously.
- Car driver with business insurance (essential for community visits).
