One Medical is looking for a Nurse Practitioner or Physician Assistant to join our team as an Advanced Care Partner. In this full-time, remote role, you will work live online during business hours, partnering longitudinally with approximately five Primary Care Physicians to own and manage their shared patient panels.
What You'll Do
- Maintain task category service level agreements.
- Answer clinical questions from patients and engage in clinical dialogue.
- Perform clinical decision-making based on messages, lab results, and consults.
- Own medication changes, titrations, and adjustments for chronic conditions.
- Provide video visits and phone follow-up care.
- Complete clinical paperwork such as for disability, FMLA, and home health.
- Close patient care gaps by creating orders and following up as needed.
- Process consults and provide related follow-up management and care.
- Make referrals to and collaborate with Clinical Case Management.
- Provide urgent visit patient scheduling.
- Provide continuity of care longitudinally.
- Participate actively in the care team, including real-time Slack communication, attending huddles, presenting cases, and providing support.
What We're Looking For
- 5+ years of experience as a Family Nurse Practitioner or Physician Assistant.
- Deep practice experience and proficiency in remote or virtual medical care.
- Experience in chronic disease care management, metabolic disease expertise, patient education, and analyzing CGM and EKG data.
- Expertise in managing a high-risk patient caseload.
- Must be currently licensed in California.

- The confidence to problem-solve, manage, and own 'outside the box' situations with patients and colleagues.
Team & Environment
You will work with a team of PCP peers, a clinical supervisor, and the Clinical Case Management team of Care Navigators and nurses. You will report to the Medical Director of the Advanced Care Partner Program.
