Deliver outpatient pulmonary and primary care to adult patients with chronic respiratory conditions such as COPD, asthma, and sleep apnea. Engage in remote patient monitoring, conduct clinical evaluations, and work closely with supervising pulmonologists and care teams to enhance patient outcomes through an integrated, technology-supported care approach.
Responsibilities
- Deliver outpatient pulmonary and primary care for adults with chronic respiratory diseases including COPD, asthma, chronic cough, and other related conditions.
- Perform patient assessments, follow-up visits, and coordinate care in partnership with supervising pulmonologists.
- Interpret pulmonary diagnostic results such as spirometry, pulmonary function tests, chest X-rays, and other relevant clinical data.
- Oversee treatment plans, adjust medications, and provide patient education focused on managing respiratory diseases.
- Collaborate with respiratory therapists, nurses, care coordinators, and clinical support staff to ensure comprehensive care delivery.
- Maintain accurate and timely documentation in the electronic health record system.
- Follow established clinical protocols, quality standards, and regulatory requirements in all aspects of patient care.
Requirements
- Holding an active Nurse Practitioner or Physician Assistant license in Tennessee.
- Possessing at least 1–2 years of clinical experience in pulmonary medicine, internal medicine, family medicine, or a closely related field.
- Demonstrated comfort with outpatient care delivery models.
- Exhibiting strong clinical decision-making, effective communication skills, and a patient-centered care philosophy.
- Proven ability to work effectively within a multidisciplinary healthcare team.
- Proficient in using electronic medical record (EMR) systems.
Nice to Have
- Prior experience in pulmonary, respiratory, or chronic disease management settings.
- Familiarity with remote patient monitoring, telehealth platforms, or digital health technologies.
