Provide advanced clinical care as a Nurse Practitioner within a remote, patient-centered model dedicated to managing chronic kidney disease (CKD) and end-stage renal disease (ESRD). Focus on comprehensive health evaluations, comorbidity management, and care coordination to reduce hospitalizations and support long-term patient well-being.
What You'll Do
Conduct thorough health assessments that evaluate medical, emotional, social, and financial factors impacting patient health. Collaborate with nephrologists and care teams to implement treatment plans, interpret diagnostics, and manage medications using prescriptive authority. Lead clinical coordination efforts to ensure alignment across providers and services, supporting holistic patient management.
Work remotely with a self-directed schedule during weekdays, designed to accommodate patient needs and partner availability. Occasional travel may be required for team meetings or clinical engagements.
Requirements
- Active, unrestricted Nurse Practitioner license in New Jersey
- DEA certification or ability to obtain within 90 days
- Current CPR/BLS certification
- Minimum of two years of experience as a licensed NP
- Valid driver’s license and personal auto insurance
- Reliable high-speed internet and dedicated home office setup
- Proficiency with Microsoft Office (Word, Excel, Outlook)
- Demonstrated clinical judgment and experience managing complex, chronic conditions
- Strong problem-solving skills and a proactive approach to patient and team success
Preferred Qualifications
- Five or more years as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology
- Background in value-based care, population health, or Medicare Advantage programs
- Experience completing Health Risk Assessments and reducing hospital readmissions
- Familiarity with Cerner or comparable electronic health record systems
