What You'll Do
In this role, you’ll provide continuous, patient-centered care for individuals managing chronic illnesses, particularly chronic kidney disease (CKD), end-stage kidney disease (ESKD), and congestive heart failure (CHF). You’ll oversee a dedicated patient panel, focusing on improving clinical outcomes, reducing hospital admissions, and supporting long-term wellness.
You’ll conduct comprehensive assessments, interpret diagnostic results, and develop individualized care plans in collaboration with interdisciplinary teams. Your responsibilities include guiding patients through complex care transitions, including dialysis initiation, transplant evaluation, or conservative management pathways. You’ll also lead shared decision-making conversations, including advance care planning, ensuring patients’ values guide their treatment.
Working across home visits, telehealth platforms, and partner clinics, you’ll use technology to reduce administrative load and enhance care coordination. You’ll ensure accurate clinical documentation, support risk adjustment coding, and close gaps in care related to preventive health and quality metrics.
Requirements
- Master’s degree in Nursing or a related field
- Minimum of two years as a Family or Primary Care Nurse Practitioner, with clinical focus in nephrology or cardiology
- Active state licensure and national board certification
- Current DEA registration or eligibility to obtain within 90 days of hire
- Valid driver’s license and reliable transportation for regional patient visits
- Proficiency with Electronic Health Records (EHR) systems
- Current BLS or CPR certification
- Basic computer literacy, including Microsoft Office and stable internet connectivity (minimum 3.8 Mbps down / 3.0 Mbps up, latency under 60 ms)
Preferred Qualifications
- Experience managing advanced CKD (Stages 3–5), ESRD, and dialysis modalities including hemodialysis, peritoneal dialysis, and home hemodialysis
- Background in managing medication regimens, fluid status, and dialysis-related complications
- Proven success in care transitions between hospital, outpatient, and dialysis settings
- Skill in educating patients on kidney disease progression, dialysis options, and transplant pathways
- Wound care certification
- Experience with audio-visual telehealth platforms
