Theoria Medical is looking for a Nurse Practitioner to provide high-quality, patient-centered primary care services for residents at skilled nursing facilities. Our organization is dedicated to serving patients across the care continuum with an emphasis on post-acute care, and we are committed to expanding nationally with talented individuals.
What You'll Do
- Round and manage the clinical care of the population at your assigned skilled nursing facility (SNF).
- Prescribe medications and treatments for residents residing at SNFs.
- Provide high quality, patient-centered, comprehensive primary care services.
- Complete timely documentation of all patient care encounters.
- Be available for phone and secure messaging consultations with facility nursing staff.
- Complete documentation in Theoria’s proprietary electronic health record system, ChartEasy™.
What We're Looking For
- Valid PA/NP license in the state where you will practice.
- Valid NP/PA degree from an accredited institution.
- Active DEA license (If you are a new graduate, Theoria will assist with your application).
- Familiarity with EMR systems and charting practices.
