About the Role

Provide high-quality virtual primary care to patients, including diagnosis, treatment planning, and ongoing management of acute and chronic conditions through telehealth platforms.

Responsibilities

• Conduct virtual patient consultations using secure telehealth technology
• Evaluate and diagnose acute and chronic medical conditions
• Develop and adjust individualized treatment plans
• Prescribe medications in accordance with state regulations
• Order and interpret diagnostic tests
• Document patient encounters in electronic health records
• Coordinate care with specialists when necessary
• Educate patients on preventive health and wellness
• Manage follow-up appointments and treatment progress

• Support patient engagement in their care plans
• Respond to patient messages within defined timeframes
