Centene is hiring a NICU Care Manager (RN) to join our team focused on transforming community health. In this role, you will develop, assess, and facilitate complex care management activities for members with primarily physical needs, creating personalized care plans and providing education to achieve high-quality, cost-effective healthcare outcomes.
What You'll Do
- Evaluate member needs, barriers to care, and social determinants of health, focusing on member-identified priorities to recommend plans for the best outcomes.
- Develop ongoing care plans and collaborate with providers to identify specialists and community resources to address unmet member needs.
- Identify problems and barriers to care and provide appropriate care management interventions.
- Coordinate between the member, family, caregivers, and the care provider team to ensure adequate, appropriate person-centered care.
- Provide ongoing follow-up and monitoring of member status and progress towards goals, revising care plans as necessary.
- Offer resource support to members and care managers for local services like housing, employment, and independent living.
- Facilitate care management and collaborate with providers to ensure timely member access to needed care or services.
- Perform telephonic, digital, home, or other site outreach to assess member needs and collaborate with resources.
- Collect, document, and maintain all member information and care management activities for compliance.
- Provide and facilitate education to members and families on disease processes, care gaps, provider instructions, and healthcare benefits.
- Provide feedback to leadership on opportunities to improve care quality and delivery in a cost-effective manner.
What We're Looking For
- A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
- 2 – 4 years of related experience.
- RN - Registered Nurse State Licensure and/or Compact State Licensure required.
- A Compact RN License OR California State RN License.

- Salary Range: $56,200.00 - $101,000.00 per year.
