Oak Street Health is looking for a Mobile Operations Coordinator for our House Calls Program serving patients in York, Lancaster, Allentown, and Reading. You will serve as an integral part of our in-home care team, coordinating logistics, ensuring wrap-around clinical care, and providing direct telephonic support to patients and families.
What You'll Do
- Prepare daily visit schedules, including routing and logistics planning.
- Answer incoming calls, voice mails, and escalate patient needs.
- Complete patient appointment confirmation calls and update patient information in the electronic medical record.
- Conduct outbound calls, including outreach and scheduling for new patients.
- Monitor incoming faxes and distribute them to appropriate care teams.
- Manage the patient panel registry.
- Coordinate patient transitions back to a center as needed.
- Request patient medical records.
- Monitor patient admissions and schedule post-discharge visits.
- Follow up on Durable Medical Equipment (DME) and Home Health orders.
- Follow up on specialty appointments, including arranging transportation when necessary.
- Participate in daily huddles and weekly interdisciplinary team meetings.
- Perform other duties as assigned.
What We're Looking For
- Strong computer skills: Ability to quickly navigate and use multiple computer programs.
- Professional phone etiquette.
- US work authorization.
Nice to Have
- Bilingual skills are a plus (Spanish, Polish, Russian, or other languages spoken in our communities).
- A medical background is preferred.
- Experience in a customer service setting is strongly preferred.
- Some college is preferred.

You will be part of a Care Team consisting of a Provider, Medical Assistant/Scribe, Behavioral Health Specialist, and Mobile Operations Coordinator, working within the House Calls Program's in-home care team.
