Total Life is seeking an Illinois-licensed Physician Assistant or Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our telehealth practice. You will provide psychiatric care exclusively to the geriatric population, focusing on follow-up medication management, cognitive assessments, and the development of initial care coordination plans.
What You'll Do
- Conduct telehealth follow-up visits for psychiatric medication management in geriatric patients.
- Monitor treatment response and adjust medications with attention to age-related risks, polypharmacy, and medical comorbidities.
- Perform cognitive screenings and assessments, such as MoCA or MMSE.
- Develop initial care coordination and behavioral support plans.
- Collaborate closely with nursing staff who provide ongoing patient monitoring and support.
- Communicate with the supervising psychiatrist as needed.
- Maintain timely and accurate documentation within the EMR.
What We're Looking For
- Active Illinois license and certification as a Physician Assistant or PMHNP.
Nice to Have
- Experience in geriatric psychiatry, long-term care, or dementia care.
- Experience with cognitive assessment tools and behavioral management strategies.
- Bilingual proficiency in English and Spanish.
- Established relationships within your local community.
