Responsibilities
- Conduct in-person outreach and enrollment conversations with eligible community members
- Drive same-day registrations and maximize conversion opportunities in the field
- Guide individuals through registration, intake, and enrollment processes
- Own weekly enrollment, registration, and conversion goals
- Maintain disciplined follow-up to move referrals and leads into enrolled members
- Identify and escalate barriers preventing enrollment or engagement
- Support member retention and re-engagement efforts when appropriate
- Execute enrollment activities at assigned partner sites, community locations, and outreach events
- Represent Zócalo Health professionally within community settings
- Build trust quickly and effectively with community members
- Support execution of Zócalo-hosted events designed to generate enrollment opportunities
- Maintain flexibility to work in field-based environments, including evenings and weekends as needed
- Accurately complete registrations, documentation, and enrollment workflows across operational systems
- Maintain timely and accurate activity tracking within Athena and other required platforms
- Follow established operational workflows, SOPs, compliance expectations, and documentation standards
- Communicate operational issues, field insights, and workflow barriers in real time
- Work closely with Market Growth Managers to support community-based enrollment strategy
- Contribute to a high-performance, enrollment-focused team culture
- Maintain strong communication regarding field activity, conversion challenges, and operational needs
Requirements
- 1–3+ years of experience in community outreach, healthcare enrollment, public health, sales, canvassing, customer engagement, or field-based roles
- Comfortable initiating conversations and engaging community members in public settings
- Experience working toward performance goals, outreach targets, or enrollment metrics preferred
- Strong communication and relationship-building skills
- Organized, reliable, and comfortable working independently in field environments
- Comfortable using technology and documentation systems
- Bilingual English-Spanish required
- Reliable transportation and willingness to travel locally
- Flexible schedule, including evenings and weekends as needed
Nice to Have
- A strong communicator who knows how to build trust quickly in diverse communities
- Comfortable with field-based outreach and performance-driven environments
- Experience with Medi-Cal, ECM, Community Supports, or community-based healthcare programs
- Experience with Athena health or comparable systems
- Experience with field outreach, enrollment support, canvassing, or community activation work
- Familiarity with local community resources and populations served
