About the Role

Vynca is hiring an LVN Case Manager to serve as our clients' primary point of contact, coordinating care across all providers to ensure alignment on client needs. In this role, you will manage client cases, coordinate benefits, provide education, and facilitate timely access to care.

What You'll Do

  • Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services.
  • Oversee the development of client care plans and goal settings.
  • Offer services where the member resides, seeks care, or finds most accessible, including office-based, telehealth, or field-based services.
  • Connect clients to other needed social services and supports.
  • Advocate on behalf of the client with health care professionals.
  • Utilize evidence-based practices like Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
  • Conduct outreach and engagement activities to facilitate linkage to the ECM program and log activity in the CRM.
  • Evaluate client progress and update SMART goals.
  • Provide mental health promotion.
  • Arrange transportation (e.g., ACCESS).
  • Complete all documentation, including outcome measures within established timeframes.
  • Maintain up-to-date patient health records in the EMR and other business systems.
  • Complete monthly reporting to ensure program compliance.
  • Attend training as assigned.

What We're Looking For

  • Active, unrestricted California Licensed Vocational Nurse (LVN) license.
  • Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time, with the possibility of evenings and weekends.
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
  • Working knowledge of government and community resources related to social determinants of health.
  • Excellent oral and written communication skills.
  • Positive interpersonal skills.
  • General computer skills and a working knowledge of Google Workspace, MS Office, and the internet.

Work Mode

This is a remote position open to candidates residing in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
