Role Summary
Provide essential care coordination as a licensed LPN or LVN in a fully remote setting, supporting patients enrolled in Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) programs. This contractor position emphasizes patient engagement, health tracking, and education, delivered through daily communication and digital tools.
Key Responsibilities
- Guide patients through enrollment in RTM or CCM programs using a secure online portal.
- Conduct daily phone calls and send text messages to monitor health status and maintain consistent engagement.
- Collect and record patient-reported health data, including medical history updates, current symptoms, and progress notes.
- Deliver personalized health resources such as guided stretches, mindfulness content, and light physical activities.
- Ensure all patient information is handled with confidentiality, in full compliance with HIPAA and internal policies.
- Attend regular training sessions and team check-ins to stay aligned with protocols and updates.
Requirements
- Active LPN or LVN certification required.
- Minimum of three years of clinical or remote healthcare experience.
- Comfortable using digital platforms including Zoom, Slack, and web-based enrollment systems.
- Strong organizational abilities with a track record of managing multiple priorities.
- Clear, empathetic communication skills tailored to patient support.
- Proven ability to work independently with initiative and accountability.
- Knowledge of HIPAA standards and a commitment to patient privacy.
- Reliable internet connection, functional laptop with camera and microphone, and a quiet workspace for full-time duties.
Preferred Background
Experience in neurology, orthopedics, physical medicine, rehabilitation, or sports medicine is an asset.
Work Environment

Compensation & Growth
