Practicebetter is looking for a Licensed Clinical Social Worker (LCSW) to join our team in Texas. In this role, you will collaborate closely with an interdisciplinary clinical team to deliver comprehensive, patient-centered palliative care to individuals with serious illness. You will be critical to the value-based care model, addressing psychosocial, emotional, and practical needs to support care coordination and improved quality of life.

What You'll Do

Conduct comprehensive assessments evaluating psychosocial distress, mental health needs, caregiver burden, and social determinants of health.
Clinically interpret assessment results to identify priority needs and appropriate interventions within the palliative care model.
Facilitate serious illness conversations and advance care planning with patients and families.
Provide clinical recommendations and escalate to physicians and advanced practice providers per established protocols.
Develop, document, and contribute clinically appropriate interventions within individualized care plans.
Participate in and contribute clinical insight during interdisciplinary rounds and case conferences.
Deliver short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers.
Serve as a clinical resource by identifying psychosocial barriers and recommending evidence-based interventions.
Utilize Tuesday Health’s electronic systems and tools to document clinical assessments, interventions, and care plans.
Establish and maintain therapeutic, trust-based relationships with patients and families.
Collaborate with the multidisciplinary team to improve quality of life, reduce avoidable utilization, and support holistic outcomes.
Support the team through care team debriefings and periodic memorial rounds.

What We're Looking For

Master’s in Social Work (MSW) from an accredited program.
Active, unrestricted Licensed Clinical Social Worker (LCSW) license in Texas.

Nice to Have

Clinical experience in palliative care or hospice, including experience with advance care planning and serious illness conversations.
Experience working within an interdisciplinary care model, collaborating with physicians, advanced practice providers, nurses, care managers, health plan partners, and community-based resources.

Work Mode
