Denova Collaborative Health LLC is looking for an Independently Licensed Clinical Therapist to deliver critical diagnostic assessments and tailored treatments to those facing mental and emotional challenges. In this hybrid role, you will counsel individuals and groups, focusing on prevention and addressing a wide array of issues, within a company culture that promotes a 'whole person' approach and collaboration.
What You'll Do
- Collaborate with the patient and their natural supports to develop and implement an effective service plan.
- Explain available clinical options and identify advantages or disadvantages.
- Provide clinically appropriate psychotherapy services in accordance with ethical codes, rules, and regulations.
- Serve as the point of contact for coordination and communication with other systems where clinical knowledge of the case is crucial.
- Assist in screening and coordinating integrated services for patients.
- Provide clinical oversight to ensure provision of all covered services on the service plan.
- Make referrals to community services as appropriate.
- Coordinate services and supports between inpatient and outpatient settings.
- Continuously evaluate treatment effectiveness through ongoing assessments, modifying the service plan as necessary.
- Participate in individual & group supervision meeting AZ Board of Behavioral Health Examiners and organizational standards.
- Maintain client records.
What We're Looking For
- Degree in Counseling, Marriage & Family Therapy, or Social Work.
- Current or pending Arizona license (LPC, LCSW, LMFT, Psy.D).
- Exceptional teamwork, communication, and multitasking skills.
- Proficiency in mental health diagnosis, treatment, and evaluation.
- CPR/First Aid certification.
- Valid Arizona fingerprint clearance.
- Ability to work within an EMR system.
- Must be available to work on-site once a week.

- Experience with Advance MD EMR system.
