CVS Health, through Signify Health, is hiring a per diem Nurse Practitioner or Physician Assistant to conduct comprehensive In-Home Health Evaluations. You will engage directly with individuals to bridge gaps in care, providing flexible and personalized services in the comfort of their own homes.
What You'll Do
- Conduct in-home health evaluations, including medication review, medical history, and physical assessment.
- Perform additional diagnostic tests as ordered by health plans, such as diabetic eye exams or spirometry.
- Utilize an iPad and an intuitive clinical workflow for all evaluations.
- Travel locally and within your licensed states as required for visits.
- Conduct virtual visits when needed.
- Provide all indicated Signify Health services, including Diagnostic & Preventive Services products.
What We're Looking For
- Active, unrestricted license(s) for Nurse Practitioner or Physician Assistant in the coverage area.
- Board certification.

Work Mode
This is a local role requiring travel within Monterey, San Benito, Santa Clara, and Santa Cruz counties.
