Responsibilities
- This position focuses mainly on hormone health - including predominantly Bioidentical Hormone Replacement Therapy (BHRT), thyroid conditions, and some medication management
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Develop personalized treatment plans based on patients' needs and medical conditions
- Prescribe nutraceuticals and/or medications and monitor their effectiveness
- Collaborate with other healthcare professionals to coordinate patient care
- Provide counseling and education to patients on disease prevention and healthy lifestyle choices
- Maintain accurate and up-to-date patient records using electronic health record systems
- Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine
Requirements
- NP/APRN – with a concentration in Functional Medicine, Hormone Health, Anti-Aging or Lifestyle Medicine
- Must have (or be able to obtain) an IL APRN and IL DEA licenses and any remote location determined
- Ideally full-time, but must be able to work at least 16-20 hours per week (including 1-2 evening shifts)
- Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers
- Energetic and outgoing personality with a desire to help people
- Patient first attitude and ability to consistently provide a 5-star level of service
- Knowledge of HIPAA regulations and ability to maintain patient confidentiality
Nice to Have
- Formal training and experience practicing Bioidentical Hormone Replacement Therapy (BHRT) strongly preferred
- Providers based in AZ, CO, FL, IL, NC, OH, PA, TN, TX, and VA are preferred

Compensation
Starting base salary guarantee of $130K (depending on experience), as well as bonus/revenue share opportunity.

Additional Information
- Active IL APRN and IL DEA licenses required to start
- Evening shifts required (1-2 per week)
