Responsibilities
- This position focuses mainly on hormone health - including predominantly Bioidentical Hormone Replacement Therapy (BHRT), thyroid conditions, and some medication management
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Develop personalized treatment plans based on patients' needs and medical conditions
- Prescribe nutraceuticals and/or medications and monitor their effectiveness
- Serve as a collaborating physician for other providers
- Collaborate with other healthcare professionals to coordinate patient care
- Provide counseling and education to patients on disease prevention and healthy lifestyle choices
- Maintain accurate and up-to-date patient records using electronic health record systems
- Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine

Additional Information
- Active Illinois Medical license and IL DEA license required to start employment
- Providers based in AZ, CO, FL, IL, NC, OH, PA, TN, TX, and VA are preferred
- Must be able to work at least 16-20 hours per week (including 1-2 evening shifts)
- We manage all marketing, staffing, practice management, billing and reimbursement, as well as provide all necessary equipment
