Home Care Providers of Texas is hiring a Home Health Registered Nurse (RN) to join our forward-thinking team. You will deliver skilled nursing care in patients' homes in the NW Houston, TX area, leveraging AI-driven technology to reduce administrative tasks and dedicate more time to direct patient care.
What You'll Do
- Deliver high-quality, comprehensive nursing care in the patient's home according to physician orders and the individualized plan of care.
- Collaborate with the interdisciplinary care team to develop, implement, and update personalized care plans.
- Administer nursing treatments and manage clinical activities to support optimal health outcomes.
- Continuously assess patient progress and service effectiveness, communicating findings with the care team, patient, family, physician, and case manager.
What We're Looking For
- Active RN License (TX/Compact)
- Active CPR Certificate
- Home Health Experience – Prior experience in geriatric home health care
- Self-starter with strong organizational skills
- A compassionate care approach
- Valid Driver’s License
