Sierra Home Health and Hospice is seeking a Home Health Physical Therapist to join our dedicated team in Truth or Consequences, NM. We are looking for a passionate and patient-focused professional to deliver personalized care in a community that values relationship-driven healthcare. You'll work independently with the support of our tight-knit clinical team.
What You'll Do
- Deliver physician-ordered physical therapy services in patients' homes.
- Evaluate patient function and create individualized care plans.
- Conduct initial assessments, reassessments, and plan of care revisions every 30 days.
- Supervise Physical Therapy Assistants and Home Health Aides as needed.
- Educate patients and caregivers on exercises and home care routines.
- Collaborate with the care team and physicians on treatment adjustments.
- Prepare timely and accurate clinical documentation, including OASIS.
- Contribute to quality initiatives, in-services, and patient care conferences.
What We're Looking For
- Current licensure as a Physical Therapist in New Mexico.
- Graduate of an accredited Physical Therapy program.
- Strong assessment and care planning skills.
- Ability to work independently and manage a home-based caseload.
- Excellent communication with patients, families, and team members.
- Valid driver’s license and reliable transportation.
Nice to Have
- Prior home health experience.
