Responsibilities
- Serve as the named Primary or Secondary Care Partner for a set of California-based Assisted Living communities
- Run the outbound workflow including callbacks, follow-ups, confirmations, and proactive outreach
- Review and reconcile medication records using AI tooling and apply pharmacy training to flag drug interactions, dosage concerns, and discrepancies
- Keep facility census data accurate and current across AllCare's systems
- Confirm provider schedules (Primary Care, Psych, Podiatry, Diagnostics) directly with facility contacts
- Handle patient documents, delivery coordination, and cycle updates with pharmacy partners on behalf of facilities
- Follow up on tasks created by the AI Concierge, resolve open issues, and check in proactively with administrators
- Coordinate across AllCare's Routing, Clinic, and Launch Lead teams to ensure requests are fulfilled on time
- Escalate issues with judgment — know when to resolve, when to loop in your Lead, and when to pull in the Director
Requirements
- Pharmacist with clinical training and patient-facing experience
- Must have pharmacy background for medication reconciliation, pharmacy coordination, and clinical judgment on drug-related questions
- Strong written and spoken English skills
- Ability to work US Pacific hours from Egypt
- Must be comfortable living on a US work calendar including US holidays
- Accountability and ownership mindset — 'don't pass the monkey'
- Process-minded with accurate logging and task completion
- Customer-first orientation with belief that a single call can make or break a relationship
