Allegheny Health Network is seeking a Health Coach to join our Pittsburgh team in a full-time capacity. You will assist with population health management through patient outreach and care coordination, participate in clinical visits to support providers, and coach patients on self-management. This role demonstrates leadership in communication, team development, and data management to enhance patient care and office operations.
What You'll Do
- Actively participate in population health outreach by identifying patients with gaps in care using analytic tools and registries, contacting them, and facilitating closing those gaps.
- Ensure pre-visit planning is completed by performing chart and data reviews ahead of patient visits and preparing for patient arrival.
- Demonstrate expanded rooming protocol by assisting the provider in addressing care gaps and appropriately activating standing orders.
- Accurately perform clinical transcription during the patient encounter by documenting elements of the visit under direct provider supervision and assisting with computerized provider order entry.
- Use a motivational interviewing approach to coach patients toward self-management, self-efficacy, and activation, assess current state, collaboratively set goals, and reinforce clinical care plans.
- Perform registry and quality data review and conduct population outreach to close gaps in care.
- Oversee the identification of high-risk patients and facilitate complex care management referrals, collaborating with care team nurses to coordinate care for rising-risk patients.
- Review and assess with care team members to achieve population health goals, improve patient access, and reduce avoidable emergency department visits.
- Clean and sterilize instruments per established standards, ensure adequate inventory of medical supplies, and stock all patient treatment areas appropriately.
- Perform in-house testing following established standards and policies.
What We're Looking For
- High school diploma or equivalent.
- Medical Assistant Certification or Licensed LPN.
- Completion of an accredited course of study in medical assisting or two years of relevant experience.
- CPR certification.
- Act 34 Criminal Background Clearance Certificate.
- Act 33 Child Abuse Clearance Certificate.
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.

You will be a member of the health care team, working collaboratively to support patient care and office operations.

This position is onsite in Pittsburgh.
