Position Summary The Transform Diabetes Care Pod RN serves a critical function in enhancing member health trajectories by establishing telephonic connections with diabetes patients. This nursing professional will navigate individuals through strategic care interventions, evaluate medical requirements, and promote comprehensive wellness. The role emphasizes addressing systemic health barriers, providing targeted education, facilitating resource connections, and implementing referrals to specialized care management programs. This position supports strategic health service development and implementation, focusing on high-quality, economically efficient care models. Utilizing advanced clinical reasoning and analytical thinking, the RN coordinates integrated physical and behavioral health services, collaborating with healthcare providers to ensure comprehensive psychosocial support access. The fundamental objective is optimizing resource allocation and driving meaningful health improvements. The RN will leverage sophisticated motivational interviewing techniques to comprehensively assess member health status, maintaining strict adherence to organizational and regulatory protocols. Proactive collaboration with supervisory and team personnel is encouraged to overcome challenges and achieve programmatic objectives. Key Responsibilities: - Execute telephonic member engagement for diabetes population health management - Deliver comprehensive educational support promoting self-management strategies - Diagnose and mitigate social health determinants affecting care delivery - Generate targeted care management and community resource referrals - Facilitate interdisciplinary care coordination - Implement nursing protocols consistent with regulatory standards - Meticulously document interactions within electronic medical records - Consult supervisory personnel to resolve complex member health scenarios Required Qualifications - Minimum three years clinical experience managing chronic condition populations - Demonstrated diabetes education expertise - Active, unrestricted Registered Nurse licensure; multi-state credentials preferred - Certified Diabetes Educator (CDE/CDCES) certification mandatory - Advanced proficiency in Microsoft Office ecosystem and electronic medical records Preferred Qualifications - Superior communication and interpersonal capabilities - Exceptional member engagement and conversational health assessment skills Education - Bachelor of Science in Nursing degree required Anticipated Weekly Hours 40 Time Type Full time Pay Range Compensation spectrum: $54,095.00 - $155,538.00 Base salary determination incorporates multiple factors including professional background, educational credentials, geographical considerations, and relevant professional attributes.
