Responsibilities
- Deliver one-on-one health coaching sessions via phone or video
- Help patients understand and manage chronic conditions such as diabetes and hypertension
- Collaborate with clinical teams to align coaching strategies with medical care plans
- Use motivational interviewing techniques to encourage behavior change
- Monitor patient progress using digital health platforms
- Document coaching interactions in electronic health records
- Educate patients on nutrition, physical activity, and medication adherence
- Support patients in setting realistic and measurable health goals
- Identify barriers to care and work with patients to develop solutions
- Promote preventive care and routine health screenings
- Encourage self-efficacy in managing long-term health conditions
- Follow clinical protocols and organizational guidelines
- Respond to patient inquiries within established timeframes
- Participate in team huddles and care coordination meetings
- Maintain patient confidentiality and comply with privacy regulations
- Stay current with best practices in chronic disease management
- Provide feedback to care teams on patient engagement trends
- Assist with care transitions and post-discharge follow-up
- Engage patients who are hesitant or disengaged from care
- Use data to tailor coaching approaches for individual needs
- Support patients in navigating health system resources
- Report patient progress to supervising clinicians as needed
- Complete required training modules on time
- Adhere to performance metrics related to patient engagement
- Contribute to quality improvement initiatives
