About the Role
The health coach will work directly with patients who have chronic diseases, offering ongoing support, education, and behavior change strategies tailored to individual needs. Coaches use digital tools and structured programs to guide clients toward better health habits and measurable improvements.
Responsibilities
- Deliver one-on-one coaching sessions to clients managing chronic health conditions
- Help clients set realistic health goals and create actionable plans
- Monitor client progress using digital health platforms and documentation tools
- Encourage adherence to treatment plans and lifestyle modifications
- Provide education on nutrition, physical activity, stress management, and medication adherence
- Use motivational interviewing and behavior change techniques
- Maintain consistent communication through video, phone, and messaging platforms
- Adapt coaching style to meet diverse cultural and personal backgrounds
- Collaborate with clinical teams to support coordinated care when applicable
- Follow clinical protocols and organization guidelines for patient safety
- Complete required training on health coaching methodologies and software systems
- Ensure all client interactions comply with privacy and confidentiality standards
- Report key outcomes and engagement metrics as required
- Participate in quality improvement initiatives
- Stay current with best practices in chronic disease management and behavioral health
Nice to Have
- Certified Diabetes Care and Education Specialist (CDCES)
- Experience using electronic health records or patient management systems
- Fluency in Spanish or another language spoken by client populations
- Background in nursing, dietetics, or allied health professions
- Knowledge of Medicare and Medicaid chronic care management programs
Compensation
Contractor role with 1099 pay structure; compensation based on service delivery and client engagement.
