PurposeCare is seeking a Field Nurse (RN) to join our mission-driven team. This role blends direct patient care with clinical leadership and oversight, ensuring our home care services are safe, compliant, and patient-centered. What You'll Do
- Conduct patient assessments during home visits to evaluate health status and care needs
- Review, revise, and update individualized care plans to ensure safe and effective care
- Identify changes in patient condition and coordinate appropriate interventions
- Ensure care delivery meets regulatory and clinical standards
- Conduct in-field supervisory visits for Home Health Aides (HHAs) and Personal Care Assistants (PCAs)
- Complete required VA assessments at Start of Care and bi-annually
- Evaluate caregiver competencies and provide guidance to support high-quality patient care
- Ensure staff adherence to individualized care plans and agency policies
- Serve as a key liaison between patients, caregivers, and agency staff
- Communicate patient condition changes promptly to appropriate team members
- Support collaborative care coordination to ensure positive patient outcomes
What We're Looking For
- Active and unrestricted Registered Nurse (RN) license in the state of practice
- Minimum 1 year of acute care nursing experience
- Strong knowledge of home care regulations and compliance standards
- Excellent written and verbal communication skills
- Highly organized with strong clinical documentation accuracy
- Ability to work independently while collaborating with a care team
