Role Overview:
As a Care Coordinator, you'll be the critical bridge connecting clients to their optimal health. You'll be the go-to person who ensures every client receives white-glove service, navigating their personalized wellness path with precision and genuine care.
Core Responsibilities:
- Client Relationship Management:
- Be the warm, responsive first point of contact for clients, delivering empathetic support that makes them feel heard and valued.
- Build trust-based relationships that turn clients into long-term advocates.
- Expertly guide clients through onboarding, health assessments, and care plan implementation.
- Care Plan Coordination:
- Partner closely with healthcare providers and internal teams to craft and continuously refine personalized care plans.
- Proactively track client progress and swiftly address any emerging needs or plan adjustments.
- Seamlessly coordinate appointments, follow-ups, and communications between clients and providers.
- Operational Support:
- Maintain meticulous, up-to-date client records in our internal systems.
- Continuously optimize processes to enhance care coordination efficiency.
- Manage scheduling and logistical details for client consultations with precision.
- Data Management and Reporting:
- Analyze client health metrics to uncover meaningful insights and improvement opportunities.
- Develop comprehensive reports on client outcomes and service effectiveness.
- Ensure ironclad compliance with privacy regulations and confidentiality standards.
- Educational Support:
- Empower clients with resources that demystify their care plans.
- Stay ahead of health and wellness trends to provide cutting-edge guidance.
Requirements
- Minimum of 3+ years of experience in healthcare coordination, client management, or a related field.
- Background in the health and wellness industries is highly preferred.
- Exceptional communication skills that can build instant rapport and trust.
- Organizational ninja with top-tier time-management abilities.
- Tech-savvy professional comfortable navigating healthcare platforms and CRM systems.
- Proactive problem-solver with an unwavering commitment to exceptional client care.
