Foundation Health is hiring a Family Physician

Responsibilities

  • Evaluate and manage acute and chronic medical conditions across all age groups.
  • Deliver routine health assessments, preventive screenings, and immunizations to support long-term wellness.
  • Educate patients on nutrition, physical activity, lifestyle choices, and mental well-being to promote disease prevention.
  • Monitor patient health over time and adjust treatment strategies based on progress and changing needs.
  • Perform minor surgical interventions, wound management, and common outpatient procedures.
  • Order and analyze lab results, imaging studies, and other diagnostic tests to inform clinical decisions.
  • Address urgent medical concerns with prompt evaluation and stabilization, referring to emergency services when necessary.
  • Coordinate referrals to specialists and ensure continuity of care for complex health issues.
  • Collaborate with nurses, pharmacists, therapists, social workers, and other healthcare providers for integrated patient management.
  • Support long-term management of chronic illnesses such as diabetes, high blood pressure, asthma, and mental health conditions.
  • Guide patients through the healthcare system to access treatments, medications, and community resources.
  • Engage in public health efforts like vaccination drives and health education campaigns to improve community outcomes.
  • Maintain precise, secure, and up-to-date medical records in accordance with legal and ethical standards.
  • Follow federal, provincial, and territorial regulations and professional guidelines in medical practice.
  • Build lasting patient relationships to ensure consistent, trusted, and personalized care.
  • Deliver care that respects cultural, ethnic, and personal differences within a diverse patient population.
