What You'll Do
Lead virtual health evaluations for patients in their homes, ensuring thoughtful, individualized care through secure digital platforms. Begin each day by reviewing assigned cases and setting up the necessary technology to support seamless remote visits.
Collaborate closely with in-home clinical staff who perform on-site assessments, using real-time input to guide patient interactions. Build trusting relationships with individuals from varied backgrounds, gathering essential health information and conducting comprehensive risk and cognitive screenings.
Evaluate medication regimens, recommend adjustments when needed, and support preventive care strategies. Coordinate blood testing logistics, connect patients with specialists, and link them to community services that enhance well-being. Accurately document all findings and care plans in compliance with clinical standards.
Requirements
- Current Family Nurse Practitioner license in California
- Board certification as a Family Nurse Practitioner with active status in TX, LA, or OK
- Demonstrated professionalism, empathy, and commitment to patient confidentiality
- Proficiency with laptops, mobile devices, and electronic charting systems
- Experience using HIPAA-compliant messaging tools for care coordination
- Strong written and verbal communication abilities
- Ability to work autonomously and collaboratively within a distributed team
- Successful completion of a national background check and license verification
