Role Overview
As a Family Nurse Practitioner, you will play a central role in delivering remote clinical care to patients across Ohio. Your work will focus on virtual wellness visits, enabling individuals to receive high-quality health assessments from the comfort of their homes. You'll use secure digital tools to guide care coordination and support in-home clinical teams as they conduct patient visits.
Key Responsibilities
- Begin each day by reviewing assigned patient cases and ensuring all technology is operational
- Conduct comprehensive virtual health risk evaluations and cognitive screenings
- Review current medications and recommend adjustments as needed
- Deliver clear, compassionate education on health management and preventive care
- Collaborate with in-home clinical staff to assess vital signs and coordinate necessary procedures such as blood draws
- Support care planning, including specialist referrals and connections to local resources
- Maintain accurate, timely documentation of all clinical interactions
- Ensure seamless coordination between virtual and in-person care components
Qualifications
- Current Family Nurse Practitioner license in Ohio
- Board certification as a Family Nurse Practitioner
- Demonstrated commitment to patient confidentiality, professionalism, and ethical care
- Proficiency with digital charting systems and HIPAA-compliant communication platforms
- Strong interpersonal skills with the ability to build trust across diverse populations
- Ability to adapt quickly in a fast-paced, evolving care model
- Successful completion of a national background check and license verification
Work Environment
